A Special Report On Antiretroviral and Viagra
Question: A 60 year old man is interested in starting sildenafil (Viagra). Current medications include 3TC (lamivudine, Epivir), d4T (stavudine, Zerit) and nevirapine (Viramune). Will Viagra interact with any of these medications?
Answer: Because sildenafil is principally metabolized by the liver's cytochrome P450 enzymes (particularly the 3A4 isoenzyme), it can have significant interactions with drugs that are either inducers or inhibitors of these enzymes, including the protease inhibitors and the non-nucleoside reverse transcriptase inhibitors. Although the use of sildenafil with antiretroviral agents has not been studied, enzyme inducers such as nevirapine might decrease sildenafil blood levels. Patients on nevirapine should receive the usual 50 mg starting dose of sildenafil, but might require higher doses to achieve the same results. In contrast, drugs that are enzyme inhibitors reduce the clearance of sildenafil and lead to higher blood levels. For example erythromycin, a specific 3A4 isoenzyme inhibitor, increases the area under the curve of sildenafil by 182%. the protease inhibitors and delavirdine are also 3A4 isoenzyme inhibitors. Ritonavir is a very potent enzyme inhibitor and might have a greater effect on sildenafil that erythromycin. Nelfinavir, indinavir, and delavirdine are probably slightly less potent enzyme inhibitors. Health care providers should use sildenafil with caution in patients who are taking protease inhibitors or delavirdine, starting with a low dose (25 mg) and monitoring carefully for toxicities such as headache, flushing, dyspepsia, and vision abnormalities. This low dose does appear to be effective in some patients treated with both ritonavir and sildenafil. Sildenafil itself is a weak inhibitor of cytochrome p450 enzymes. Because of very low peak plasma levels, it is unlikely that sildenafil will alter the clearance of other drugs metabolized by these enzymes (including non- nucleoside analogues and protease inhibitors).

How safe is it:Fisting can be very safe if you know what you are doing. You can't force it or you may damage rectal tissues. The rectal tissues are not as delicate as 'wet paper towels' circa The Joy of Gay Sex. It is very important that the fister has short and smooth fingernails.
HIV transmission: HIV transmission is very minimal with just fisting, if you clean up correctly with a disinfectant soap afterward and do not contaminate yourself (i.e. rub your eyes in the middle of fisting). Some men like to use gloves and this gives an added protection. The downside to gloves is that some people develop Latex sensitivity.
Hepatitis transmission: Hepatitis transmission can also occur during fisting but this is of low risk. Like with HIV, cuts in the skin on the hand of the fister is a way for the virus to enter. Gloves can protect from most of these problems. Doctors/nurses and other health care professionals have to work with secretions of HIV and hepatitis patients, studies have found that gloves and hand washing cut down the rate of transmission tremendously. Also, it is suggested by the CDC that 'homosexuals' be vaccinated against Hepatitis B, if they don't already have the protective antibodies and the vaccination of Hepatitis A is probably a very good idea also. You can find out your Hepatitis status, including Hepatitis C from your health care provider with a simple blood test.
Other problems that could occur: Most of the problems that occur during fisting is people not really know what they are doing. A new 'fistee' should have an experienced 'fister' to help him through some of the potential problems. Toys are the leading cause of rectal damage NOT fisting.
How can I learn to relax: fisting is a form of meditation to me, not just an act of expanding the anus and rectum. TRUST is the most important thing you must have when you are trying to be fisted. Forcing your ass to do something will not work. Most of the muscles in fisting are involuntary smooth muscles. Your 'head has to be in the right place' for things to work. Talk to experienced men in fisting and they can tell you volumes about this.
How can I stretch myself faster: There are several ways to help. You can play with graduated toys, i.e. sized from small to larger. There are inflatable buttplugs and the like. Again, you have to go slow and pain is your clue that you are going too fast. There are other things that can also help, like muscle relaxers, anesthetics, analgesics, etc., but again, you need to have a good knowledge of what you are doing or play with an experienced man who can teach you.
Why is there is almost always a bit of blood that occurs at the start of fisting that goes away quickly: A little bloody show is not unusual, it can be a slight irritated hemorrhoid to an irritated anal wart to something more serious like an anal fissure. If you just get a little pink Crisco that goes away with a quick flush then it is probably not a problem. Anything more than that and you should let your health care provider know. If you can't talk to your health care provider about such things, then find one that you can talk to.
About hemorrhoids: Hemorrhoids or piles are enlarged veins in the wall of the anus (internal hemorrhoids) they are usually a consequence of prolonged constipation or, occasionally diarrhea. Uncomplicated hemorrhoids are seldom painful; pain is usually caused by a fissure. An anal fissure is a break in the skin lining the anal canal (mucosa) and the main symptom is bleeding. In first-degree hemorrhoids, bleeding is at the end of defecation and is the only symptom. First-degree hemorrhoids never appear at the anus. In second-degree hemorrhoids the hemorrhoid protrudes beyond the anus (the so called 'first sphincter') as an uncomfortable swelling but returns spontaneously. Third- degree hemorrhoids remain outside the anus and need to be returned by pressure.
How can I prevent hemorrhoids: First-and second-degree hemorrhoids may respond to bowel regulations using a high-fiber diet with fecal softening agents. (Metamucil, Citracel, Fibercon.) On treatment of First and second degree hemorrhoids it is interesting to note that 'Forceful dilation of the anus under general anesthesia is also effective.' Well, fisters don't need general anesthesia but this is why that regular fisters have a much lower incidence of first and second degree hemorrhoids. Third-degree hemorrhoids, on the other hand (no pun intended) often require surgery especially if they become strangulated, producing severe pain and further enlargement. External hemorrhoids are either prolapsed internal hemorrhoids or-more often- perianal hematomas (an accumulation of blood within the tissues that clots to form a solid swelling around the anus) or the residual skin tags remaining after a perianal hematoma has healed. Perianal hematomas can be seen with punch fucking if the fistee is not relaxed enough and as a 'hicky' with rough rimming.
How long after a hemorrhoidectomy do I have to wait until I start training again: The anal and rectal tissues heal remarkable fast. I would suggest that you wait until your check up with your surgeon and he does not see that anything is still in the active phases of healing. Undoubtedly he will give you a much longer time to wait then you really need to for medicolegal reasons.
If I have a hemorrhoidectomy, is it possible to leave my anus a bit looser: The procedures for hemorrhoidectomy does not involve the muscles directly and therefore there should be no difference. If you are a bit looser it may be due to some temporary nerve damage that will heal or a complication of permanent nerve damage that you do NOT want.
Other than HIV and hepatitis are there any other issues or dangers for tops: Since a top is indirectly dealing with stool he has all the possibilities of any infectious agent that can be transmitted by stool, such as bacteria, viruses, and parasites. Most of these pathogens (disease causing germs) are transmitted by the so called 'fecal-oral route.' That is, there need to be contamination that somehow gets into your mouth. This is often seen when a food handler with an infection does not wash his hands and handles your food after it is cooked. (the most common cause of travelers diarrhea).
How safe is it: It is a body secretion but not as high on the list to infect as semen, or stool.
HIV transmission: see above discussion under HIV
Hepatitis transmission: see above discussion under hepatitis
Other problems: Other problems that could occur if the person has a urinary tract infection that could be transmitted to his partner.
Pissing up someone's ass: Since this is a bodily secretion it has some chance of transmitting disease, although less then semen and stool
What fluids go through the fastest: Balanced or isotonic fluids (similar salt concentrations) will cause faster urination than just plain water. This is also safer in that you will not be loosing excess salts from your body reserves.
Is there more a risk with stronger piss than weaker piss: Stronger urine has more waste products that some can be absorbed through your partners rectum. In general more dilute urine is usually preferred for play to more concentrated urine.
Enemas "A quantity of fluid infused into the rectum through a tube passed into the anus. An evacuant enema (soap or olive oil) is used to remove feces. A therapeutic enema is used to insert drugs into the rectum."
How safe is it: enemas if done correctly are very safe. They have been given for centuries.
Problems that could occur: Some problems that can occur is that too much water is given and too many times. This can cause dissension of the colon and possible damage, and loss of electrolytes (salts) that can lead to serious metabolic problems, such as palpitations, muscle weakness, etc. If plain water is given the problem of salt loss is much greater. To correct for this a solution of water and table salt to the taste of your tears is about the right amount. What fluids clean the fastest: just above body temperature water that is balanced with salt, a so called isotonic solution or Normal saline (0.9%)
Caster Oil - oral: Caster oil is an irritant laxative that will work very fast but may leave your rectum too irritate to play.
Magnesium citrate - oral: this is a hypertonic solution (high salt content) that again will work very fast buy may cause colic or cramps during play.
Mineral Oil - oral: Mineral oil is good as a stool softener and takes days to work unless you take an overdose. Larger amounts of mineral oil will block your ability to absorb vitamins, minerals, and other nutrients. If you play often this is a good way to get malnourished.
Mineral Oil - anal: This can be useful to clean the bowel and coat it so that excess salts are not lost. This will not work very fast and will be quite messy. This is the so called 'oil retention enema' that is given in hospitals and is mainly for hard stool in the rectum. The usual dose is about 6 oz and this would never be enough to prepare for fisting. Using more than the usual amounts have some of the same hazards as above with the oral mineral oil, i.e. blocking the rectum from it's natural function in digestion.

How safe is it: scat can be safe if the play is limited to non-mucosal surfaces, i.e. intact skin since the skin is a very good protective barrier against germs.
HIV transmission: is high with this type of play esp. if feces from someone else enter the rectum or mouth.
Hepatitis transmission: the fecal-oral route is the main mode of transmission of the hepatitis viruses esp. type B.
Other than regular dangers with scat, are there any other problems that might occur if you fist with shit as a lube: The possible problems that could occur if feces are present when fisting occurs is that the mucosa (mucus membrane, inner lining the rectum/colon) could break down and bacteria can enter the deeper layers of the colon, causing an abscess or even further and entering the abdominal cavity and causing peritonitis. Many people with normal diets have certain foods that do not break down even when in the rectum, such as particles of nuts, corn and other foods that are indigestible to humans. These particles can act as irritants and cause the break in the mucosa.
I heard about something that is called "peritonitis". It's some kind of disease and it's something that you can have if you have too much and heavy ass play. What is peritonitis?
(Surgeon's answer)Peritonitis refers to swelling of the peritoneum-the covering of the abdominal cavity. It is normally a sterile cavity and gets infected once the internal organs of the abdomen are injured that bacteria spill over into it. Heavy assplay can cause deep tears/ulcers that can cause spillage of fecal matter and bacteria into the peritoneal cavity leading to peritonitis. It is a surgical and medical emergency since left unattended, it can lead to sepsis (blood poisoning) and ultimately death.

In your opinion, is there a significant difference in the level of risk in terms of STD and drug-resistant HIV strain exposure in not ejaculating loads up the ass as opposed to what one would be exposed to in precum during bareback fucking? If you do one, might as well be doing the other?
(Surgeon's answer)No difference whatsoever. Anal sex is considered a high risk activity whether with or without ejaculation since tiny tears occur that can serve as portals for the transfer of the HIV virus-either anal/rectal tears or penile tears.
(Internist's answer)Pre-cum can carry the same viral load of Hiv as semen. The difference would be in the inoculum (the amount of infectious material) i.e. most men make 1 to 2 teaspoons of semen while only a few drops of 'pre- cum.'
And what are your thoughts concerning oral sex? All things equal, if you're sucking cock raw (which everyone seems to do), does the rate of exposure increase that dramatically if you gulp that load down?
(Surgeon's answer)The cocksucker especially if he takes in the load has the higher risk of getting infected since you are ingesting directly the other person's body fluid which has a high viral load. In contrast, the one who gets sucked has a lesser risk since studies have shown saliva to have minimal viral load. However, should tears occur on the cock during sucking, then the risk of getting HIV increases dramatically.
(Internist's answer)The risk is higher in sucking cock and ingesting semen compared to ingesting and not taking it or not swallowing it. To make a heterosexual corollary. Coitus interuptus (pulling out before cumming), still has a risk of pregnancy, not as high as coitus that is not interrupted. This would also apply to bareback anal sex and Hiv from the man doing the fucking.
And while I'm asking, I love to eat butt. Not about to give it up. And I've been very fortunate all these years regarding contracting intestinal difficulties, warts, etc. I generally know my partner's health, and never lick an unwashed butt. However, I'm wondering if there are any additional precautions that can be taken to reduce the risks of oral-anal contact.
(Surgeon's answer)ANSWER: The risk involved in this kind of activity is similar to that in kissing. If there are open wounds such as anal fissures/ulcers or oral ulcers, the virus can be transmitted but relatively the risk is let's say medium. What one might get more often is the transmission of parasitic infections and this can be avoided by general hygienic practices in foodhandling.
(Internist's answer)Studies have show that rimming is still consider 'high risk behavior." I would disagree with my colleague in this and NOT put oral- anal sex in the 'moderate risk behavior' category but keep it in the high risk behavior category. The main reason is contamination with feces. So a washed and douched out ass has a lower risk then an unwashed rimming esp after a bowel movement. But both would still be a 'high risk' behavior.
What is ethyl chloride? What are its effects? Is it legal? Is it dangerous? Where can I get it?
(Surgeon's answer)Ethyl chloride is a clear liquid anesthetic that vaporizes on exposure to air. It is used as a brief topical anesthetic in the field of ENT. Like any anesthetic, it numbs the area and causes vasodilatation. When inhaled, its effect is similar to poppers which is why some handballers use it to relax. Regarding its other effects and adverse reactions, I'm not aware. Any anesthetic should be used with caution since it acts directly on the brain when inhaled. When used topically like along mucous membranes, its effect is very brief and localized.
(Internist's answer)I agree with the surgeon's opinion on ethyl chloride. Caution should be taken with inhalants, esp this one, compared to poppers. Sudden rising from a laying down position and standing and esp running can cause a sudden faint. In stronger inhalants such as "glue sniffing" this causes a number of deaths in apparently healthy children every year. Ethyl chloride is only available on Rx and I cannot think of a legitimate reason that a patient should have this at home. (that is to say, the pharmacist will question why a doc is writing a Rx for this, and know that it can be a 'drug of abuse,' this would be reason to report the health care provider to the Board of Pharmacy. Outside of the US, you can probably but it without a RX)
(Additional personal comment from PigMaster) I never thought I would be including my own input on this area of the Pig Pages, but this issue I feel I need to since I am a victim of Ethyl Chloride A.K.A. Hard Drive and Gaz. As a sufferer of insomnia for days at a time, I would use Ethyl as a step to "cross over" into sleep. One night after going through two 4 oz cans, I started suffering many medical problems - some permenant - some serious. The bottom line is, using Ethyl is "huffing" just like glue or any other fumes to get high. While short term use may not be damaging at all, heavy episodes of use WILL hurt you.
I am interested in becoming totally bladder incontinent...is there a safe sane way to do this so I have to wear my diapers?
(Surgeon's answer)ANSWER: The only way I know to become incontinent is by cutting the nerve to the detrussor muscle of the bladder--that is surgical incontinence more often a complication of pelvic surgery rather than an elective procedure. Medically, no idea.
(Internist's answer)In medicine there are many drugs to help a person STAY continent. I cannot think of anything off hand that has an adverse affect of causing incontinence that is at all safe to take as a 'recreational drug.' I have heard of some men into catheter play of keeping a Foley (balloon) catheter in their penis for long periods of time and urine just flowing out without accumulating in the bladder. The only problem with this is that a urinary tract infection from outside germs going up the catheter could become a real problem. If you were to use a catheter and wear a diaper the changes of infection would be lowered. I don't know if having a catheter and not being able to control your urine is the same as being incontinent, in a sexual speaking manner.
What general precautions / advice would you suggest concerning the following: 1) Cleaning of latex devices... are you aware of the cleaner Simple Green... I am curious that if my nephew is cleaning his devices with this product would cause problems. I used it on mine instead of Betadine and seemed to have no adverse effect. 2) The best lubricant for use with latex devices for individual, personal use... I have read E-mail stating that any natural oil (Canola, Corn, etc.) is safe and non-toxic... and very very slick for more beastly self indulgences... (Surgeon's answer)Waterbased lubricants work best for latex devices since oil-based lubes corrode and breaks down the rubber in these devises making them brittle. For cleaning latex devices, use only tap water and mild soap.
(Internist's answer)Since latex devices are used and cleaned much more in surgery than in Internal medicine I would defer to the surgeons opinion. Betadine can also stain pink flesh colored latex toys. This is often why men by black latex toys.
I see by reading your Swine Flu area that drinking your piss can be worse that someone else's. Is there an amount of your piss that one can drink with out getting a kidney problem? Like 1 glass a day or once a week or once a month etc, or if it has been diluted and not the first piss of the morning? Any help would be great. I have check with my doctor and I am disease free. But I really do love the taste of my piss. I would never use a toilet if I could get by with out it.
(Surgeon's answer)Drinking one's own piss can result in an autoimmune reaction. Urine is waste material discarded by the body and is identified by one's own body as such. Ingestion of this same waste material may give the body a wrong signal later after a prolonged period identifying it as a foreign material so that what normally is a normal by-product of your body is now identified as foreign. The reaction known as an autoimmune reaction can have grave consequences. Taking another person's piss does not induce this reaction to occur although getting STD is a real risk in watersports.
(Internist's answer)As previously posted, I had talked about the possibility of developing an autoimmune disease. I agree completely with the surgeons answer but would add that this is fairly rare. So even if you have been drinking your own urine from a young age this does not mean that you will develop an autoimmune disease but that you have a higher risk. I would discuss this with your health care provider and have him/her check that you have not started an autoimmune process.
My left testicle is sore when my lover sucks it a little too hard, if it rubs on my underwear the wrong way or sometimes for no reason at all. I have been to the doctor and he tells me that there is nothing wrong. Can you give me any suggestions?
(Surgeon's answer)There are many possibilities for pain in the testicles. It could be subacute/quiescent orchitis or epididymitis. Was a urine exam done? If so what was the result. Any change in size or color of the testicle? A blood clot can also cause pain in that area especially after some heavy play.
(Internist's answer)This is possibly a surgical problem and I defer to my learned colleague. Orchitis: inflammation of a testis. Epididymitis: inflammation of the duct along the back of the testis that stores and transmits sperm. This is very important that this is followed by a health provider because cancer of the testis often starts in a similar manner.
Syphilis levels - Several years ago, a routine blood test revealed a long-standing syphilis infection. I was treated twice with a series of penicillin injections and while the titre was notably reduced, it never hit zero. I was told that this occurs occasionally and that there's no need to worry. However, I do worry now and then- what do you think?
To quote from the Merk manual under Syphilis: Surveillance: The importance of tests to confirm a permanent cure should be explained to the patient before treatment. Examinations and quantitative STS (Serologic tests for syphilis) should be made monthly for 6 mo. and every 3 mo. thereafter for an additional 18 mo. in primary and secondary syphilis. Following successful treatment, lesions heal rapidly and serologic titers fall, the reagin test usually becoming negative within 9 to 12 months. The specific tests, such as the FTA- ABS (fluorescent treponemal antibody-absorption) and the TPHA (Treponema pallidum hemagglutination assay), usually remain positive for years or even for the rest of the patient's life. The CSF(Cerebral spinal fluid) should be examined after 1 yr of surveillance. If the VDRL (Venereal Disease Research laboratory) remains positive for more than 1 yr or if the titer starts to rise, re-treatment should be considered. If re-treatment is required it should be with double the original dose of antibiotic. If all the clinical and serologic examinations remain satisfactory for 2 years after treatment, the patient can be reassured that cure is complete and permanent and he need not return.
In Syphilis in HIV-Infedcted patients: Nontreponemal quantitative test should be repeated at 1, 2, and 3 months and thereafter at 3-month intervals until titers have stabilized. If titers have not fallen twofold by 3 months in primary syphilis or 6 months in secondary syphilis or if there is a fourfold or greater increase in titer, the patient should have a spinal fluid examination and be re-treated.
I wrote out this so that you can copy it and bring it to your doctor. I don't know what stage, what test etc... were done and you can see that it does matter. My advise is get your medical records together and make an appointment with your health care provider address your concerns.
Cleaning Out - I'm not the most experienced fisting top and I've recently been wanting to bottom. I've done it a couple of times a few years ago but really didn't prepare and subsequently stopped doing it. What I need to know, besides the need for practice, is what is the best way to clean myself out prior to a fisting session. I've done enema hoses in the shower for pleasure but have heard that it can dry out the rectal walls. When should I stop eating (or should I)?
Each bottom fister has his own recipe for cleaning out it seems. I suggest eating light the day of play. Drinking plenty of fluids like GatorAide to replenish the salts that you will lose while taking enemas. I suggest that you use a small (not more than 1/2 quart) enema a couple of times then give yourself a rest. Check again with one or two tap water enemas. Just before play give yourself a very short one, called a proctoclysis (cleaning of just the anal-rectal area, NOT the descending colon like before), then you should be good to go. Some men who never seem to rinse clean use some Immodium to stop the trickle of small intestinal contents into the large bowel. You will have to see what your system needs and ask experienced bottoms what you should try.
Long Term Cockrings I have used cockrings for short periods of time, but would like to wear them for extended periods of time to have a fuller basket. Are they safe to wear for longer periods, and how tight before they get unsafe?
Pain is your guide. Use the snap on unless you are very sure of how big you get when erect. You can do damage to yourself if you use a steel ring, get turned on and can't get it off or loose the erection. It could cause an erection problem later on.
If You're Going To Eat Shit I have been eating some shit off and on now for about 3 years. I only got sick once, hepatitis-A, a very mild case, and recovered after a gamma- globulin shot. I'm HIV negative, and in excellent health. Question 1: Is there any way to "prepare" oneself to ingest a LOT of shit?
You should have the Hepatitis A and B vaccine, as well as pneumococcal (pneumonia) and a yearly influenza vaccine. You may even want to take the thyphoid series of vaccines. A good idea would be to have your partner tested and have his stool tested for infections. If it is your own stool then you should also have a culture and sensitivity, ova and parasite test of the stool to make sure you do not carry any diseases.
Question 2: Does the gamma globulin shot "protect" me from further hepatitis infections? If so, for how long? Can you go get another BEFORE getting sick?
Gamma globulin gives you about 6 weeks of partial protection. See above for recommendations. Gamma globulin is meant as an emergency drug, not for routing use.
Poppers 1. Is there any relationship between anal sex (and therefore prostate stimulation) and prostate cancer?
No, there is no relationship as far as causal. As a matter of fact, gay men find their prostate cancer earlier because they self detect it or their partner does. Same for rectal cancer.
2. Does poppers lower your T-cell count, and if so, for how long?
There were many studies of this during the 1980's at the center where I trained, New York University. Poppers do not lower your white cell count or make you more likely to get Hiv or (as was once thought) give you AIDS.
3. Are poppers safe for fisting? Doesn't it alter one's pain thresh-hold?
It does not lower your pain threshold to any extent. It is safe for fisting.
4. Can you overdose on poppers, how much is ok, and is there an antidote?
You can overdose on anything but if you are using a bottle then it would be very difficult. The antidote is to breathe fresh air if you take too much. The effects will pass in a few minutes.
1) My partner and I are intrigued, curious and seriously tempted to play with scat. Piss is already a big part of our relationship. We have done scat externally, but I badly want to eat it from my boyfriend. I have already rimmed him a number of times after he took a shit and it just made me want to do it all the more. Scat taken by mouth can transmit other infections besides Hiv and Hepatitis. For safer play you should both have a stool culture and test for parasites. If you don't let your health care provider know what is going on he will do the 'routine screening' and may miss certain infections or parasites that you may have. Depending on the excretion of the drugs that you are on will influence the taste of your urine. If the major route of excretion of any drug is through the kidneys then it will influence the taste of your urine. You may also have some of the drug or metabolites (by products) that may be inactive or even active in your urine. Your partner may not want to take your medication, even indirectly through your urine. You need to talk to your health care provider on all the drugs that you are on and their route of excretion. By the way, the package insert on the drugs have this information, under Pharmacokinetics and Metabolism.

There are several things you can do to calm down your hemorrhoids before play. A high fiber laxative supplement, (bran, psyllium, polycarbophil and methylcellulose) such as Citracel, Metamucil, Fibercon etc.can be used when you are not getting ready to be fisted. The use of Preperation-H or Anusol can be used after every bowel movement and at bedtime, on those nights where you are not going to play. Use of Tucks pads can clean and help soothe the tissues after a bowel movement. Also sitz baths (sitting in very warm water, like in a bathtub) can help calm down the tissues also; this can be done several times a day when the hemorrhoids are active. You may need the Rx forms of Prep-H or Anusol HC which has hydrocortisone in it. The main thing is never to let yourself get constipated and not to use irritant laxatives (ex- lax) and cathartics.(Epson salt). Rather, you should try milk of magnesia, colace, and fiber laxatives. Glycerin suppositories, are useful if the first part of the stool is hard. Of course, increasing the fiber in your diet and drinking plenty of water are some general things to do to prevent constipation. Also to prevent constipation you need to set your "internal clock" for a regular bowel movements. Set aside a regular time each day for bowel movements. The best time is often within 1 hour after breakfast. Don't try to hurry. Sit at least 10 minutes, whether or not a bowel movement occurs. Drinking hot water, tea or coffee may help stimulate the bowel also. When you first get the urge that you need to have a bowel movement, stop what you are doing and try to have a bowel movement. Do not bear down and force the stool out, that is a good way of exacerbating your hemorrhoids or causing new ones. Many people suppress this so called 'gasto-colic reflex" (nerves in the stomach are stimulated and send a signal to the colon) this urge will pass but the next time it comes it may not be as strong and you may not feel you need or have the time to respond. This is one of the major reasons humans get constipated. Also, certain drugs can cause or exacerbate constipation, you would have to check the package insert on the medication you are taking then talk to your health care provider about if any of the routine treatments will interfere with your medication.

Caverject or Muse (alprostadil) by penile injection or urethral suppository works by direct action on the corpus cavernosum. The use of poppers does not have a significant adverse reaction. So for the men that need an erectile medication aid, if you use poppers do NOT use Viagra but try Caverject or Muse instead. If poppers are not your thing and you don't have to take nitroglycerine for your heart then Viagra may be a good choice. Since all of these are on Rx you need to talk to your health care provider about your specific medications, health problems and which drug is right for you.
I somehow like the feeling of having a very full bladder while doing sex, so I sometimes drink a lot of water in order to get one. Is it possible to damage the bladder, kidneys or urinal tracts or something when I do that and hold it for hours?
Holding your urine is not really a problem unless you get obsessive-compulsive about it. Your bladder can expand quite a bit and unless you have nerve damage in your back or pelvis the pain will let you know when enough is enough.
I sometimes engage in play (solo) where I utilize playing with 3 different sizes of Foley catheters. To date I have yet to experience and problems. I reuse the ones I have, instead of buying new ones each and every time. I always sterilize them before and after play, by washing them thoroughly with antibacterial soap and a 15" - 20" soak in a strong sodium hypochlorite, hot H2O solution. Many people have to catheterize themselves for medical reasons on a regular basis. They just need to keep their catheters clean and change them every once in a while. They do not need to be sterile. For medicolegal reasons catheterization in a hospital is a sterile procedure with disposable catheters. You sound like you are doing the right things NOT to give yourself a urinary tract infection. If you do have urinary discomfort, frequency, or foul smelling urine get your doctor to check a urine specimen and let him/her know that you do use catheters. You do not want a big work-up when it would not be needed.

I don't really know, but I'm sure there are some limits. Pain is the limit as far as I am concerned. I will tell you a funny story even though you did not ask for one. Scat is dangerous since about 99% of feces is bacteria. Now most of the bacteria is not a pathogen to man (disease causing agent), but if there is any pathogenic bacteria than this is a highly dangerous practice. If the stool only goes on unbroken skin then there is a much higher safety factor. And yes, stool can contain HIV virus as most bodily secretions can. The main point is in the inoculum (the amount of infectious agent per unit) and where it goes. The rectal mucosa is one of the most vulnerable area to any infectious agent. It has very little defensive mechanisms unlike the mouth. Again, a good point is to know the health of yourself and your partner. If you or your partner don't have an STD then you can transmit or contract one. Unfortunately, with scat, certain bacteria that would otherwise not cause disease will cause disease when in such a high inoculum is ingested or taken rectally. If it is just pig play with stool getting on the skin then a good disinfectant soap should handle that.

Pain is usually a good guide that something is wrong. With an enema it may be that you have a kink in your intestines and body movements will let the water continue to pass. The pain is usually from the dissension of the colon, it can be caused by flatus (intestinal gas) , air from fisting, or from fluid or stool such as colic. Some people can take more than a couple of liters of fluids but then their colon is larger and used to that amount. During a barium enema the average adult non-fister can handle up to about 2 to 3 liters of fluid. Also, sometimes you will get cramps due to the type of fluid that you are using in the enema, to ensure less cramping an isotonic solutions (i.e., a solution that salts match your own) or so called normal saline at body temperature is the best for this. This can be made by tasting the enema water before you use it. It should have the taste roughly of the salinity of tears. You can use table salt to get to the right amount.

You may have something simple like a few hemorrhoids that bleed when you play. You can usually feel the bigger ones but the small ones may go undetected. Using a hemorrhoid prep over the counter may help. Also, when you fist get fisted during a play session there may be a little pink Crisco, this is very common and usually not a problem. If you can slow down that first time and not be in such a hurry that may solve the problem. Also using a mild steroid prep. such as the Prep H on Rx or Anusol on Rx can help. To get the Rx you can have your doctor check to see about hemorrhoids and hopefully talk to him about your fisting habits. Also, at that time he/she can see if you have anal warts that may bleed with play or something more serious like an anal fissure that will need medical/surgical attention. If he/she doesn't respond to your questions well, you may think about finding a handball friendly MD. There is not bottom line. But my opinion as an MD of Internal medicine and gay is that it is OK if there is nothing contagious about your partner. Urine is usually sterile but you don't want do drink it if he may have a urinary tract infection. Also, drinking YOUR own piss may be more of a problem than other peoples. You can develop auto-antibodies and start an autoimmune problem with your kidneys. This is NOT AIDS and is not infectious but a problem similar to having lupus of the kidneys. This material was found when survivors of drought were studied. Drinking someone else's piss did not cause the problem but in some, drinking their own urine cause this problem to arise.

Sounds are metal rods that are used to dilate the lower urinary tract. To play with them you really need to know what you are doing. They need to be used sterilely to insure that you don't give yourself a urinary tract infection. The main thing is that you don't push them but let the weight of the sound itself advance them and gently dilate your urinary sphincter. Again, my advise is to find someone sane that know how to play with these safely. A pig MD or RN would be my choice.

Removal of anal warts can be done by chemicals, electric fulguration or cryosurgery (freezing with liquid nitrogen). The costs varies depending on method and who does it. That is, a proctologist will charge a lot more than a family practice provider. You need to talk about the method and the cost before you have anything done. My opinion is that liquid nitrogen is the easiest and best method for most cases. If the warts are not locally confined then the treatment, esp, in HIV cases may need interferon-a.
Can you tell me what health precautions a man should take before getting involved in watersports? What diseases can a man expect who receives 'piss enemas'?
First thing is to check with your health care provider that you are not contagious and talk to him/her about 'watersports.' If they give you attitude then I would suggest finding a gay friendly provider to help. With 'piss enemas' or 'golden enemas you can be infected with STD's (sexually transmitted diseases) since some semen can be present during this type of sex play. This basically would be the same exposure as with 'barebacking' but the inoculum would be much lower (amount of infection). There is a theory that the urine may kill some STD's and that 'golden enemas' would be therefore less likely way of transmitting STD's in general.

The use of a spermicidal gel would not help in barebacking unless you are trying to prevent pregnancy. The use of a lubricant that contains Noroxidol would help but that is only a minor help in combating STD's (sexually transmitted diseases). The best is to know the health of your partner and yourself and to stay 'exclusive' as much as possible when it comes to barebacking. I know that many people have other opinions but this is mine as an MD.

See the above comments on drinking urine. As far as recapturing lost vitamins and minerals it is only trace amounts and the first. Most people into water sports don't use the more concentrate part of the urine but dilute it with drinking beer and other fluids before play. I would suggest getting your vitamins and minerals from food and vitamin supplement. This is probably an ancient practice when such things where not available. As far as the 'urine of a pregnant woman able to kill some kinds of AIDS cancers', I asked a gay MD that is a perinatologist (takes care of high risk pregnancies) and is a professor. He has never heard of such a thing. I spoke to a friend online that has heard something, and it was in the first trimester urine. This is on the edge, to say the least.
